In order to redeem, please submit your claim online or by mail. This rebate offer is valid only for legal residents of the 50 United States, the District of Columbia, and Puerto Rico, who are 18 years old or older (includes APO/FPO addresses). The offer does not apply to Canadian residents. Rebate submissions must be postmarked by July 3, 2018. Not valid with other offers. Allow six to eight weeks after mailing for delivery of your prepaid card. Limit one offer per envelope and two offers per name/household/address.
